My Assessment tool is designed to take the guesswork out of the situation. It gives you hard data so you can stop operating out of fear and start moving forward with a clear, objective plan.
This interactive tool does not need to be printed out; it is an editable pdf. Check the boxes and input the section score. The total score will auto-populate. You can save it and reference it later. See sample below.
Once you download and fill out the assessment, look closely at Page 4. If your parent scores in the Moderate or High Risk categories, it means their safety—and your health—are under immediate pressure.
