This tool does not need to be printed out; it is an editable pdf - you can check the boxes on your computer. The total score will auto populate. You can save it and reference it later. See sample below.
Once you download and fill out the assessment, look closely at Page 4. If your parent scores in the Moderate or High Risk categories, it means their safety—and your health—are under immediate pressure.
