Care managers work collaboratively with physicians, nurses, therapists, social workers, facilities and other healthcare providers. They may also serve as advocates for patients, helping them navigate the healthcare system and access other services and resources.
The goal of Care Management is to ensure that patients access high-quality care and that their needs are met. Care Managers assess the patient's physical, psychological and social needs and develop care plans, coordinate services and resources, monitor patient progress and provide education and support to patients and their families.
No, not all are nurses. There are care management companies with Care managers who do not have a clinical background or social work background. There also social workers who work as care managers.
First and foremost, being able to provide actual nursing care. Most Care Managers are "hands-off" and do not have a clinical background.
By having a Nurse as a Care Manager, you have someone who is able to actually assess your clinical condition and provide nursing care such as wound assessment and dressing changes, actual medication management, supervising care aides, while having the clinical expertise and background to discuss your care with your doctor, specialist or other members of the healthcare team.
You are not dealing with someone who has to hire a wound care nurse or a nurse to come perform nursing services.
A case manager primarily focus on coordinating services and resources for clients while care managers focus on coordinating care and medical treatment.

Ask. Ask about their experience. Ask about their education and experience. Have they taken any courses in Care Management or Navigation? Not all nurses have care coordination experience. In fact most nurses don't. Just as there are Nurse Case Managers in hospitals working alongside Social Workers who are also care managers, the nurse working at the bedside many not have the experience of a Nurse Case Manager just as a Social Worker who isn't a LCSW may not have the experience to work as a therapist.

We keep up to date with you so that we can update other healthcare providers and provide the necessary information when you cannot do so. For example, we can arrange transportation to the Emergency room, provide the staff with your medical history, allergies and other pertinent information. and reach out to family member contacts to notify them of your condition and keep them updated.
Reduced Fee for a la Carte Services: You have the option to call for sick day care, one time care and other a la carte services for a much reduced fee. In addition, Care management clients enjoy a $50 reduction on postop and preop care fees.
An Nurse Advocate who is for you and by your side: Whether you need someone to accompany you or be by your side as and advocate in the Emergency room, doctor's visit, navigating specialists, or other healthcare situation, you have have one who knows you. You have a professional who can provide the expertise and knowledge for recommendations.
Check-ups: At a minimum, we check on you at least yearly to update your information and assess your general well-being.
Navigating Long-term Care: With experience on the other side doing assessments for Long-term Care insurance companies, we can help you navigate that, facility placements, checking up on your loved-one in a facility and making recommendations. Having had access to assessing people in different facilities, we can help you with the right placement. Whether you need memory care, in-home care and other services, we help you navigate that.
